Wise County Public Schools Dear Parent/Guardian:
We are pleased to be able to provide seasonal flu vaccine in partnership with your school system this year. How Much Will Flu Vaccine Cost? We will bill your insurance for flu vaccine. The Health Department will accept payments from insurance companies with which the Virginia Department of Health has a contractual billing arrangement, including Virginia Medicaid/MCOs, fee for service Medicare, Anthem and Anthem Healthkeepers, most commercial United Healthcare plans, Surest, and Piedmont Community Health Plan. The Health Department will forgive any balance the contracted insurance companies/payers do not pay. If your insurance company requests additional info from you in order to pay this claim, then you must provide that info in order for your balance to be forgiven.
• If your child does not have insurance, your child may receive influenza vaccine free of charge through the Virginia Vaccines for Children program.
• If your child has a private insurance that does not have a billing arrangement with the Health Department, or that does not cover flu vaccine, your child may receive injectable (shot) vaccine at a cost of $30.00. You must send payment in the amount of $30.00 to your child’s school.
Does Your Child Need to be Protected from the Flu? The Center for Disease Control’s Advisory Committee on Immunization Practices (CDC-ACIP) recommends all persons 6 months of age and older receive an annual seasonal flu vaccination by the end of October. Children younger than 9 years of age may need 2 doses of seasonal flu vaccine to be fully protected. If you would like your child to receive flu vaccine at school, please do the following:
1. If you school system is using Redcap online registration, please complete the online consent form by October 3, 2024.
2. If your child is under age 9 at the time of vaccination, you must provide your child’s previous flu vaccination history on the online consent form.
3. Please read and complete the consent form as follows: • There are questions to determine if your child is medically eligible to receive flu vaccine. Please answer each question. • Enter your child’s insurance information in the insurance section. Please confirm that all information is entered correctly. • Complete the consent for child’s vaccination section. Your signed or typed signature is required, in the area designated by an X. Please call your local health department if you have questions about the school flu program or completing the online consent form.
