Transition begins with elementary school. However, official transition planning begins when the IEP student turns 14. Transition is the process students and their families use to identify postsecondary goals and helps students and parents to think about life after high school. It also helps to ensure they are acquiring the knowledge and skills to achieve their postsecondary goals.
